Complex Care at Home
What is complex care?
Complex care is specialist support that is bespoke, person-centred, outcome focused and tailored to meet the needs of someone with a chronic or long-term health condition. As a result of the condition, the person requires extra assistance to manage their symptoms and daily activities to live as independently as possible within their home enjoying a higher quality of life.
Complex care can include any condition that requires clinical support, such as Parkinson’s, acquired brain injury, spinal injury, stroke, multiple sclerosis, cerebral palsy, motor neuron disease and other neurological conditions. Interventions required can include PEG feeding, Continence Care (catheter care and stoma care), ventilator care, tracheostomy care, airway clearance etc.
Type of support available
Our complex care packages are overseen by our team of clinical experts. So, whether you need 24-hour care as your condition progresses or daily visits from a local carer to support with a catheter, our clinical team will carry out a detailed assessment to ensure you have exactly the right care in place for you. Care options available include the following,
- Visiting care
We have a dedicated team of carers who can visit your home for a minimum of one hour. Frequency of calls can be arranged to suit an individuals needs for example one person may require two calls a day another person might require three or four calls a day. - Respite Care
This is where care is provided on a short-term basis, e.g. for two weeks to give the regular carer a break. - Live in care
This is an arrangement where a carer moves into the home of your loved one to provide round-the-clock care and one-on-one support as required.
