Supported Living
Our service offers Supported Living, Semi Independent Living, Outreach Support and Supported Tenancy services tailored to the needs of the individual. We provide a bespoke, robust, structured and person-centred Care and Support service on a 24-hour basis to Service Users with a diagnosis of:
- Primary Diagnosis of Learning Disability
- Autism
- Asperger’s Syndrome
- Mental Disorders
- Challenging Behaviour
- Complex Needs
- Comorbid Diagnosis
In addition, we provide a high standard of accommodation and supportive environment for residents aged 16 to 24 years. We aim to ensure young people are able to develop and maintain respect, self-reliance and self-care skills and enable them to make informed, age-appropriate decisions, concerning their present and future so they can achieve a successful transition into independence and adulthood.
Our model of care is based on the principles of Positive Behavioural Support. Our care packages are fully tailored to meet your individual needs. We offer bespoke care and specialist complex care support that is person centred, planned, proactive and coordinated for people with a diagnosis of learning disability living in their own homes in order to prevent hospital admission.
